Absence Forms
Faculty, Post Docs, and Teaching Academic Staff, who are planning on traveling or being absent while being paid by University administered funds, should complete a Request To Be Absent form, available from the Dept. Administrator or from the Math Dept website, under Information, click on Forms. If your absence occurs during class times, remember to arrange coverage for your classes.
